
Enrollment - Youth Members
For children ages 8-18 (Cloverbud, 5-8). STEP 1: Contact your local Extension office to connect to a local club and to receive enrollment information. STEP 2: Contact Club Leader and, using the online enrollment system, create a family household profile, and add each member both adult and youth to the household. STEP 3: Complete the Medical/Liability Release Form (one per child). This form is available on the enrollment site. If necessary you may print the form and return it completed to the Extension Office. STEP 4: Choose projects. New members, please select "Welcome to 4-H" and only one or two other projects.
Adult Volunteers
Volunteers must complete be enrolled on the enrollment site and pass a background check.
STEP 1: Complete the three Adult Volunteer forms listed below.
- Volunteer Application Form
- Disclosure & Consent Form
- Adult Medical/Liability Release Form Available on Enrollment Site
STEP 2: Submit completed forms and a check for $10 (made payable to "Bernalillo County 4-H") to the Extension office.
STEP 3: Once the background check has been completed, you will be sent an acceptance letter be with instructions.
STEP 4: Complete the online enrollment, including the "Working with Minors" training found on the enrollment site under the "Clover Academy" link.
